The Conners 4 is available in three lengths: full-length (109–118 items), Short (49–53 items), and a 12-item ADHD Index (see Table 1.1 for a comparative overview).

The Conners 4 can be rated by parents (or guardians), teachers, and by youth using the Self-Report. The parent ratings reveal the youth’s behavior at home and in other environments where the parent can observe the youth. Teacher ratings reveal observations of the youth’s social, emotional, and academic functioning in a school setting. Self-report ratings can supplement parent and teacher reports by providing the youth’s perspective on their own functioning and provide a gauge of their level of insight into their feelings, thoughts, and behaviors. Self-reports provide valuable information about feelings and thoughts that may not be easily observable by others. The consistency of the content included in the Conners 4 scales and items across the parent, teacher, and self-report forms facilitates the comparison of information between these sources.
The Conners 4 Parent, Teacher, and Self-Report forms are available in English, U.S. Spanish, and French-Canadian. The U.S. Spanish and French-Canadian versions are linguistically and culturally sensitive adaptations of the English versions, with the cultural relevance of the items considered throughout the item development process (see chapter 6, Development).
